The under 6s GP visit card covers:
Free GP visits
Assessments at age 2 and 5
Out-of-hours urgent GP care
Care for children with asthma
Medication costs and hospital charges are not covered

Medical conditions and costs; If you have any long-term medical conditions, you must include a medical report from your doctor or consultant.
If you have high medical costs and you do not qualify for an over 70s card, you can apply for a general medical card or GP visit card here!
GMS & Doctor Visit Card - Apply Here!
Use this form to apply for a medical card or a doctor visit card. The form will need to be signed and stamped by your GP before you send it off.
We can do this without an appointment for any of our registered patients
If you are a new patient applying for a Medical Card/Doctor Visit Card with us, you will need to have a quick chat with one of our GPs before we sign the form for you. There is no charge for this service.
Long Term Illness Scheme - Apply Here!
If you are suffering from certain long-term conditions you should use the this form to apply for a long-term illness book which will ensure that you do not have to pay for drugs, medicines and medical appliances associated with the care and treatment of your condition.
The form will require a signature from your doctor. Conditions covered by the Long-term Illness Scheme are:
● Mental handicap
● Mental illness (for people under 16 only)
● Diabetes insipidus
● Diabetes mellitus
● Haemophilia
● Cerebral palsy
● Phenylketonuria
● Epilepsy
● Cystic fibrosis
● Multiple sclerosis

Carers Allowance is a payment for carers who look after certain people in need of full-time care and attention. You may qualify for Carer’s Benefit instead of Carer’s Allowance if you leave the workforce to care for a person in need of full-time care and attention. Disabled Persons Parking Permit - Click Here!
Disabled Person's Parking Permits or Cards (also known as European Parking Cards or Disabled Parking Badge) are available to people living in Ireland with certain disabilities and those who are registered blind, whether they are drivers or passengers. The parking card can be used by a disabled person in any vehicle in which he or she is travelling. This means that a disabled person who is being driven at different times by different people can bring the parking card with himself or herself and display it in the appropriate vehicle. The parking card is valid for 2 years from date of issue. Generally, the card is not issued to anyone under 5 years of age.
How to apply
- Request a parking card application form from the Disabled Drivers Association, you can email parkingcard@ddai.ie.
- Alternatively you can apply online to the Irish Wheelchair Association, or email maats@iwa.ie
- Decision makers within the issuing authorities will determine if you are eligible based on the information provided.
European Health Insurance Card (EHIC) - Click Here!
EHIC replaces the old E111 form The European Health Insurance Card (EHIC) lets you get healthcare in another EU or European Economic Area (EEA) state for free, or at a reduced cost.
It covers you if you are on holiday, or on a short-term stay. A short-term stay is generally accepted as for a period of less than 3 months. The exception to this is a student who may be considered a visitor on a short-term stay up to a full academic year.
You need one for each member of your family. It's free and there's no charge to apply.
There are different ways to apply depending on: if you live in Ireland or if you live in another European Union (EU), European Economic Area (EEA) member state, or Switzerland.
Seafarers Medical Certificate - Click Here!
Applicants for a seafarer medical certificate must be at least 16 years of age and should contact us directly to arrange an appointment. Any seafarer attending a medical examination must produce personal and photographic identification, which will be checked by the Approved Doctor.
When it is not a first medical, the previous medical certificate should also be brought to the examination. The fee should be met by the seafarer or seafarer’s employer or company
In view of the importance of meeting the eyesight standards, anyone considering a seagoing career is strongly advised to have a full eyesight test before beginning training to ensure that they meet the standards at www.seafarers.ie

Cross Border entitles Irish patients to get treatment in either a public or private setting within Northern Ireland or another EU country.

The amount that will be repaid, is the amount that the treatment would cost in Ireland or the cost of the treatment abroad if that is less. Following the appointment, patient must submit the receipts to the HSE for reimbursement.

Patient is entitled to be treated at a Private Hospital as if it were a hospital within Ireland.
